we evaluated the joint space widths of the tibio-talar joint in the neutral position, dorsiflexion and plantarflexion of the ankle joint using 3d reconstruction images of ct. there were no significant differences in the amount of change in dorsiflexion and plantarflexion relative to that observed in the neutral position. in the medial-middle and medial-posterior positions, the joint space width increased in dorsiflexion, and decreased in plantarflexion. this phenomenon may be attributable to the ligamentous structure of the tibio-talar joint. in the medial-anterior position, the joint space width was not significantly different in dorsiflexion and plantarflexion. this phenomenon may be attributable to the lack of a significant change in the plantar-flexion position.

the amount of change in the joint space width in dorsiflexion and plantarflexion relative to that observed in the neutral position was calculated. in the medial-middle position, the amount of change in plantarflexion was significantly larger than that observed in dorsiflexion. in contrast, in the medial-posterior and medial-anterior positions, the amount of change in dorsiflexion was significantly larger than that observed in plantarflexion. furthermore, in the medial-anterior position, the amount of change in plantarflexion was significantly larger than that in dorsiflexion, and in the medial-middle position, the amount of change in plantarflexion was significantly larger than that in dorsiflexion. this phenomenon may be attributable to the ligamentous structure of the talonavicular joint.

in our study, the mean values of the medial malleolus, tibial plafond and lateral malleolus were 1.08 mm, 1.62 mm and 2.04 mm, respectively. in the study by kido et al, the mean values were 1.64 mm and 2.03 mm, respectively [ 12 ]. we observed narrower joint space widths than those reported by kido et al [ 12 ], which may have been due to the differences in the patients enrolled in the studies. the mean value of the tibial plafond in our study was greater than that reported by kido et al [ 12 ], which may have been due to the differences in the measurement methodologies. the mean values of the medial malleolus and lateral malleolus reported by kido et al [ 12 ] were similar to those reported in this study.

from may to october 2013, the authors used a cirs ct/x-ray (siemens healthcare, erlangen, germany) to obtain the ankle ct images of 10 subjects. the subjects consisted of 7 males and 3 females. their mean age was 56.1 years (ranging from 33 to 65 years), with a mean body height of 168.3 cm (ranging from 157.0 to 183.8 cm), and mean body weight of 70.8 kg (ranging from 48.5 to 88.2 kg). the mean period of the images was 86.1 days (ranging from 21 to 101 days). the ankle ct images were obtained under the following conditions: (a) non-loaded condition (no weight applied); (b) neutral ankle position; (c) maximum dorsiflexion; and (d) maximum plantar flexion. after the images were obtained, the contact area and contact pressure were measured in the neutral and three positions (neutral, dorsiflexion and plantar flexion). the resulting data were compared with those obtained using 2d methods. in addition, contact pressure was compared between the dorsiflexion position and plantar flexion position.
