The use of absorbable sutures is now widely accepted in vascular surgery and has become commonplace in a number of different applications. The Food and Drug Administration has approved four types of absorbable sutures for clinical use in vascular surgery: oxidized-cellulose, polyglycolic acid (PGA), polydioxanone (PDS), and polylactic acid (PLA). This article reports results from a randomized, controlled, multicenter study comparing the use of absorbable sutures to conventional nonabsorbable sutures in vascular surgery. Postoperative thrombosis developed in three of 172 PGA suture patients. At 6 months, two of the three patients were doing well and one patient was doing poorly. Among 132 PDS suture patients, 13 had thrombosis. Thrombosis occurred in 5 of 81 PDS patients at 3 months, in 3 of 46 patients at 6 months, and in 1 of 40 patients at 9 months. Three of the 13 thrombi could be palpated and two were first detected by Doppler ultrasound. Of the two patients in whom thrombosis was detected by Doppler ultrasound, one had an acute thrombosis and the other had a chronic thrombus. This study provides evidence that PDS sutures are not as safe as PGA sutures in vascular surgery. Similar data were obtained for PLA sutures. PGA sutures were consistently safer than PDS or PLA sutures and no patient with PGA sutures had a thrombosis. Thus, absorbable sutures are safer than nonabsorbable sutures in vascular surgery.THIRD RUN
